How to fill this in
Fill in one form for one project and one phase. If you run more than one phase or more than one project, publish a separate form for each.
For each field, attach the proof document it names, or link to where a buyer can view it. A statement without its proof document is not this disclosure.

Developer disclosure form
One form for one project and phase. Attach the proof document named in each field.
Developer
Legal entity name and type (PT or PT PMA), NIB number, AHU registration number. Attach the deed of establishment, SK AHU and NIB.
Land right
State the registered right (SHM, HGB, Hak Pakai or HPL), its certificate number and holder. For a lease, state the lease term and who holds the underlying right. Attach the certificate or lease.
Zoning
State the KKPR or RDTR suitability, and that the land is not protected productive farmland. Attach the KKPR document.
Building approval
State the PBG number, and the SLF status (SLF exists only after completion). Attach the PBG and, if issued, the SLF.
Completion and handover
State the completion date, the PPJB and handover schedule, what happens if the project is delayed or cancelled, and whether you commit to a quarterly progress report.
Buyer's money
State which applies: a bank or non-bank construction guarantee, staged payments, or neither, stated plainly.
Nominee and returns statement
State that this is not a nominee arrangement, and that no return is promised or guaranteed. If a rental programme is offered, name the operator, its NIB and KBLI.
Notary, PPAT and agent
Name the notary and PPAT with their working area, and, if an agent is used, the agency's NIB and the broker's competency certificate number.
Completed projects
List your completed projects, each with its own PBG and SLF numbers.
